Flagler Cares, doing business as Volusia Cares, is seeking a compassionate and skilled full-time Behavioral Health Therapist to provide outpatient counseling and care coordination for adults experiencing mental health and substance use disorders. Many of the individuals we serve are involved with multiple systems—including criminal justice, child welfare, healthcare, and homelessness services—and have historically experienced significant barriers to accessing behavioral health care.
This position is ideal for someone who enjoys building meaningful therapeutic relationships while collaborating with community partners to address the complex factors affecting a person's recovery and wellbeing.
Most Essential Work:
- Provide one-on-one care coordination and behavioral health navigation to individuals facing complex needs.
· Individual Counseling.
- Develop and monitor individualized stabilization plans addressing behavioral health, substance use, housing, employment, and other social care needs.
- Engage with clients regularly to assess progress and remove barriers to care.
What will you be doing?
- Provide individual outpatient counseling to adults experiencing mental health and substance use disorders.
- Conduct comprehensive behavioral health assessments, including biopsychosocial evaluations and risk assessments.
- Develop individualized treatment plans in partnership with clients that reflect their personal goals and strengths.
- Utilize evidence-based therapeutic approaches such as Motivational Interviewing, Cognitive Behavioral Therapy (CBT), Solution-Focused Brief Therapy, Acceptance and Commitment Therapy (ACT), and trauma-informed care.
- Support individuals throughout their recovery journey by helping them build coping skills, improve emotional wellness, reduce substance use, and achieve greater stability.
- Coordinate care with healthcare providers, recovery programs, hospitals, law enforcement, probation, child welfare, homeless service providers, and other community partners.
- Contribute to a team culture that values compassion, innovation, accountability, and whole-person care.
What we are looking for:
- Master’s degree in counseling, Social Work or a related field
- Licensed Mental Health Counselor (LMHC), Licensed Marriage & Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Florida Registered Mental Health Counseling Intern
- Knowledge of and experience using evidence-based practices such as Motivational Interviewing, Solution-Focused Therapy, and Cognitive Behavioral Therapy
- Knowledge of co-occurring mental health and substance use disorders.
- Understanding of trauma-informed, recovery-oriented, and person-centered care.
- Strong interpersonal and communication skills
