Position Summary
We are seeking a full-time Hospice Social Worker (MSW) to join our team. This role is vital in helping patients and their families navigate emotional, social, and practical challenges that arise during end-of-life care. The Social Worker collaborates with the interdisciplinary team to ensure each patient receives comprehensive and compassionate support.
Responsibilities
- Complete initial and ongoing psychosocial assessments for hospice patients and families
- Develop, implement, and update individualized care plans
- Provide emotional support, counseling, and crisis intervention to patients and families
- Assist with advance care planning, community resources, and placement needs
- Participate in interdisciplinary team (IDG) meetings and collaborate with other hospice professionals
- Maintain accurate and timely documentation in accordance with Medicare and state regulations
Qualifications
- Master’s Degree in Social Work (MSW) required
- Current Arizona licensure (LMSW or LCSW) preferred
- Hospice experience required
- Strong communication and counseling skills
- Compassionate, empathetic, and patient-focused approach

Job Type: Full-time
Pay: $55,000.00 - $65,000.00 per year
