LVN Visiting Nurse – Hospice (Part-Time / Per Diem)

We are seeking an experienced and dependable LVN Visiting Nurse to join our hospice team.
Position Summary
The LVN Visiting Nurse provides skilled nursing visits to hospice patients in their homes, assisted living facilities, board and care homes, and skilled nursing facilities. The ideal candidate is compassionate, organized, and comfortable working independently while collaborating closely with our interdisciplinary team.
Responsibilities
- Provide routine skilled nursing visits to hospice patients.
- Assess patient condition and report changes to the RN Case Manager and clinical leadership.
- Provide patient and family education regarding hospice care and symptom management.
- Administer medications and treatments within the LVN scope of practice.
- Accurately complete documentation in a timely manner.
- Collaborate with physicians and the interdisciplinary team to ensure quality patient care.
- Maintain compliance with all hospice, state, and federal regulations.
Qualifications
- Current California LVN license in good standing.
- Minimum of one year of hospice experience required.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently and manage a flexible schedule.
- Current CPR certification.
- Valid California driver's license.
- Reliable personal vehicle with current auto insurance.
