Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Assist clients with mobility and transfers to ensure their safety
- Medication reminders
- Meal planning and light housekeeping
- Meaningful companionship and conversation
- Transportation to appointments and community activities
- Providing emotional support while preserving each client's dignity and independence
- Observe and report patient conditions promptly, noting any changes or concerns related to health or behavior
- Maintain patient confidentiality in compliance with HIPAA regulations while managing medical records and documentation accurately
Requirements
- Valid State Tested Nursing Assistant (STNA) certification or equivalent certification recognized by state licensing boards
- Proven experience working with geriatric patients, individuals with disabilities, or in long-term care environments
- Knowledge of end-of-life care practices
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to perform heavy lifting safely and use patient transfer devices effectively
- Current CPR (Cardiopulmonary Resuscitation) and First Aid certifications; Basic Life Support (BLS) training preferred
- Strong communication skills for effective client interaction and team collaboration
- Understanding of home health care regulations and compliance standards such as HIPAA and patient privacy laws
- Experience working with individuals with developmental disabilities or in hospice care is a plus
