Responsibilities
- Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor residents’ health by taking vital signs, observing patient conditions, and reporting changes to healthcare teams
- Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals tailored to dietary needs, assist with cooking, and maintain cleanliness in client living spaces
- Implement individualized care plans designed to promote independence and enhance quality of life for residents with developmental disabilities or dementia-related conditions such as Alzheimer’s or memory care needs
- Maintain a safe environment through regular cleaning, infection control practices, and adherence to safety protocols including CPR and first aid certification
- Engage clients in social activities and community outings that foster socialization and mental stimulation while respecting their preferences
Qualifications
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health care preferred
- Knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer’s care
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts
- Strong understanding of HIPAA regulations to ensure confidentiality of client information
- Certifications in CMT, CPR and first aid, Bloodborne Pathogens and DDA are highly desirable; ongoing training provided if needed
- Compassionate attitude with excellent communication skills for working with individuals with disabilities or developmental challenges.
- Valid driver’s license and reliable transportation.

- Flexibility to work various shifts including evenings, weekends, or overnight as required

- 7am to 7pm (4 days per week)
