Evaluate patients with speech, language, cognitive, communication, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs and physician orders.
Provide skilled speech therapy services in patients' homes.
Educate patients and caregivers on communication strategies, swallowing safety, cognitive exercises, and home programs.
Monitor patient progress and modify treatment plans as appropriate.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
Complete accurate, timely clinical documentation and participate in case conferences and quality improvement initiatives.
Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.
Master's or Doctoral degree in Speech-Language Pathology from an accredited program.
Current Speech-Language Pathologist license in the state of practice.
Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.
Current CPR certification.
Home Health experience is preferred but not required.
Valid driver's license, reliable transportation, and auto insurance.
