The Case Manager provides direct, trauma-informed case management services to youth in foster care and young adults participating in the Independent Living Program (ILP) at Halo Angels Children's Services. The Case Manager develops and monitors individualized transition plans, connects youth to resources across education, employment, housing, financial literacy, health, and daily living skills domains, and serves as the primary point of contact between the youth, caregivers, courts, schools, and community partners. This role plays a critical part in helping youth build the skills and supports needed for a successful transition to independent adulthood.
Key ResponsibilitiesCase Management & Direct Service
- Carry an assigned caseload of youth/young adults in foster care or ILP, in accordance with agency and state ratio requirements.
- Conduct regular in-person and virtual visits with youth to assess needs, progress, and safety, per required contact frequency standards.
- Develop, implement, and update individualized transition/independent living plans in collaboration with each youth, reflecting their goals across education, employment, housing, financial literacy, health, and daily living skills.
- Provide direct coaching and support on life skills such as budgeting, job readiness, housing search, and self-advocacy.
- Respond to crises or emergent needs, including coordinating emergency housing, safety planning, or referrals to mental health/crisis services.
Documentation & Compliance
- Complete timely, accurate case notes, service plans, and progress reports in the case management system in accordance with agency documentation standards.
- Maintain case files in compliance with state licensing regulations, funder requirements, and confidentiality standards.
- Prepare court reports, permanency planning documentation, and other required filings as needed.
- Track and report youth outcomes and program data to support quality assurance and funder reporting requirements.
Coordination & Advocacy
- Coordinate services with schools, employers, housing providers, healthcare and mental health providers, and other community partners on behalf of assigned youth.
- Attend and participate in court hearings, permanency planning meetings, family team meetings, and other required case conferences.
- Advocate for youth rights, needs, and preferences in all planning and service delivery processes.
- Collaborate with foster parents, kinship caregivers, or independent living placement staff to support consistent, coordinated care.
Youth Rights & Safety
- Inform youth of their rights, the grievance process, and available resources at intake and throughout services.
- Identify and report suspected abuse, neglect, or safety concerns in accordance with mandated reporting laws and agency policy.
- Support youth in building natural supports and permanency connections alongside independent living goals.
Team Participation & Professional Development
- Participate in regular individual and group supervision with the Program Director.
- Attend agency-required trainings, including trauma-informed care, adolescent development, and independent living best practices.
- Contribute to program planning, quality improvement efforts, and case reviews as requested.
QualificationsEducation & Experience
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field required; Master's degree preferred.
- Minimum of [1–2] years of experience in child welfare, foster care, case management, or youth services preferred.
- Experience working with youth who have experienced trauma, foster care involvement, or housing instability strongly preferred.
Skills & Competencies
- Working knowledge of foster care systems, permanency planning, and independent living/transition-age youth services.
- Strong assessment, documentation, and time-management skills.
- Excellent verbal and written communication skills, including comfort presenting in court or formal meetings.
- Ability to build rapport and maintain professional boundaries with youth and young adults.
- Proficiency with case management software and standard office/productivity software.
- Valid driver's license, reliable transportation, and ability to travel within [service area] for home visits and meetings.
Working Conditions
This position involves a combination of office-based work and regular travel to youth placements, schools, courts, and community sites within [service area].
