Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and have experience in various home settings. This role requires the ability to work collaboratively with other healthcare professionals, provide high-quality nursing care, and ensure the well-being of patients across diverse populations.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Monitor patient vital signs and report any changes in condition to the healthcare team.
- Provide education and support to patients and their families regarding health management.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Assist in procedures such as IV insertion, suturing, catheterization, and wound care.
- Participate in discharge planning and provide follow-up care instructions.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in acute care, critical care, or emergency medicine preferred.
- Familiarity with electronic medical record (EMR) systems such as AxisCare.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is advantageous.
- Strong skills in patient assessment, medication administration, and basic life support (BLS).
- Ability to work effectively with diverse populations, including those with developmental disabilities or chronic illnesses.
- Previous experience in Home Healthcare, Geriatrics, Hospice & Palliative Medicine, or behavioral health is beneficial.
- Excellent communication skills and the ability to work collaboratively within a healthcare team. Candidates who possess additional qualifications such as trauma medicine experience, long-term care experience, or familiarity with Home Health experience will be considered favorably.
