The Haven at Regency Pointe is a leading assisted living and memory care facility dedicated to providing compassionate and high-quality care to our residents. We are committed to creating a safe and supportive environment for our seniors, and we are looking for a dedicated LPN to join our team.
As an LPN in our assisted living and memory care community, you will be responsible for providing direct care to our residents, ensuring their comfort and well-being. Your duties will include:
1. Assessment and Diagnosis: Conduct initial and ongoing assessments to determine residents' needs, including physical and mental health, medication administration, and nutritional status.
2. Medication Management: Monitor and administer prescribed medications, ensuring accurate documentation and adherence to medication schedules.
3. Health and Hygiene: Assist with personal hygiene tasks, such as bathing, dressing, grooming, and oral hygiene, to promote and maintain residents' health and well-being.
4. Emotional Support: Provide emotional support to residents, their families, and caregivers, offering reassurance, companionship, and assistance in coping with challenging situations.
5. Nutrition: Monitor residents' dietary needs, assist in meal planning, and assist with food preparation to ensure proper nutrition.
6. Coordination: Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care and optimal resident outcomes.
7. Documentation: Complete accurate and timely documentation, including medical records, progress notes, and required reports.
8. Shift Coverage: Work various shifts, including evenings, weekends, and holidays, as needed to provide comprehensive care to our residents.

Licensed Practical Nurse (LPN): Must hold a valid LPN license in the state of employment.

Experience: Previous experience in assisted living or memory care preferred.
