Position Overview
The Lead Care Liaison will be responsible for managing the organization’s Care Liaison team at a Houston-based ED. This role is essential in supporting ApogeeCare’s telehealth services in ED settings, providing direct assistance to remote clinicians during assessments of older adults. This role involves program development and oversight, training, supervision, telehealth coordination and administrative tasks to ensure a comfortable and efficient experience. Additionally, the Lead Care Liaison will act as ApogeeCare’s representative in the ED, liaising between internal and external teams to coordinate comprehensive patient care. This position requires regular in-person shifts in the ED and reports directly to the Head of Care Liaisons.
Key Responsibilities
Leadership and Support:
- Working in partnership with Leadership to establish best practices, operational enhancements and telehealth standards.
- Supervise and support the Care Liaison team. As the team grows, oversee the onboarding and training of team members.
- Split time between shift-based telehealth coordination and administrative responsibilities. As the team grows, this role will focus more on management.
Telehealth Coordination:
- Identify and prepare older adults (65+) eligible for ApogeeCare telehealth services.
- Introduce ApogeeCare’s telehealth services to patients and families, complete intake and obtain informed consent for services.
- Collect focused geriatric history under the direction of remote clinicians, including cognitive, mobility and functional histories.
- Maintain accurate documentation within electronic health records (EHR) systems, such as Epic, and support clinicians by organizing patient documentation.
Technology and EHR Management:
- Maintain proficiency with computers and troubleshoot technical issues, with support from remote IT services, as they arise.
- Utilize electronic health records (EHR) efficiently for documentation, patient history and other updates, ensuring all data aligns with organizational standards.
- Set up and manage telehealth audio and video equipment (e.g., camera, tablet, audio equipment), and provide support as needed during telehealth consultations.
Liaison and Partnership Development:
- Act as ApogeeCare’s representative in the ED, fostering strong partnerships with ED staff, physicians, nurses, social workers, and other support departments to coordinate age-friendly, patient-centered care.
- Serve as the main point of contact for ApogeeCare, ensuring seamless communication and collaboration between ApogeeCare’s remote clinicians and the ED team.
- Facilitate connections with external vendors and community support services, including Assisted Living communities, Skilled Nursing Facilities, Adult Protective Services, and other agencies to coordinate post-discharge care and resources for patients.
- Engage regularly with community partners to promote comprehensive support for older adults, especially those at higher risk for adverse outcomes following ED visits.
Required Qualifications
- Registered Nurse (RN) credential with active state licensure.
- At least 3 years of experience, emergency medicine preferred.
- BLS certification required; additional geriatric care training is a plus.
- Strong interpersonal and communication skills, particularly with older adults, and the ability to work effectively in a team environment.
- Proficiency with computers, typing, and managing multiple computer programs at the same time. Willingness to troubleshoot and problem solve computer issues as they arise on their own.
- Proficiency in electronic health record systems (e.g., Epic) and familiarity with telehealth and video communication technology.
- Willingness and ability to learn or expand knowledge of age-friendly care.

Pay: $48.00 - $53.00 per hour
