At Personal Home Healthcare and Hospice we provide care needed in the patient’s home setting in order to improve health, independent functioning and prevent hospitalization.
We are seeking a full-time Registered Nurse (RN) to join our healthcare team to service patients in Monroe county and surrounding areas. General schedule is Monday through Friday daytime hours.

Responsibilities:
- Assess and establish patient treatment plans in conjunction with physician orders and patient preferences
- Help develop, coordinate and implement the Nursing Care Plan in conjunction with the multi-disciplinary careteam
- Provide skilled nursing care as ordered by the physician to patients in their home setting
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians and other healthcare providers regarding patient care
- Educate patients about health maintenance and disease prevention
- Maintain timely and accurate patient medical records
- Provide advice and emotional support to patients and their family members
- Participate in on-call duties as needed per agency policy
Requirements:
- Must be currently licensed in Pennsylvania as an RN
- Minimum one year home health and OASIS experience preferred but willing to train the right candidate
- Reliable with good communication, organization, and clinical skills
- Have a caring, compassionate, personable and collaborative attitude
- Familiarity with medical software and equipment
- A valid driver’s license in the state of Pennsylvania

Pay: $80,000.00 - $90,000.00 per year
