We are currently seeking a motivated and compassionate Physical Therapist (PT) to join a respected hospital-based rehabilitation team providing both inpatient and outpatient services. Position Summary
The Physical Therapist evaluates and treats patients across inpatient and outpatient settings to restore function, improve mobility, reduce pain, and prevent or minimize long-term physical limitations. Working as part of a multidisciplinary healthcare team, the PT delivers evidence-based care to patients recovering from injury, surgery, illness, or managing chronic conditions.
Key Responsibilities

Perform comprehensive evaluations of patients with orthopedic, neurological, musculoskeletal, and post-surgical conditions.
- Develop and implement individualized, patient-centered treatment plans based on clinical findings and physician referrals.
- Provide evidence-based interventions, including manual therapy, therapeutic exercise, gait training, neuromuscular re-education, and modality use.
- Educate patients and families on home exercise programs, injury prevention strategies, and lifestyle modifications.
- Monitor patient progress and adjust treatment plans as clinically indicated to achieve optimal outcomes.
- Maintain accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries in the electronic medical record (EMR) system.
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate patient care.
- Ensure compliance with hospital policies, state licensure requirements, and professional standards of practice.
- Participate in interdisciplinary team meetings and discharge planning activities.
Qualifications
Required

Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy from an accredited program.
- Current and active state Physical Therapy license.
- Strong communication, clinical reasoning, and documentation skills.
Preferred
-
Experience in outpatient orthopedics, inpatient rehabilitation, acute care, or hospital-based settings.
- Familiarity with EMR systems and multidisciplinary care environments.
