Basic Function
Responsible for making sure every patient seen within the practice has an organized medical record that is legible, accurate, and readily accessible, and easily retrievable. They must keep medical records in a secure place to assure limited access.
Responsibilities including but not limited to:
Make sure every patient seen has a medical record
2. Ensure that all necessary forms are in the record.
3. Files and stores records in alpha order by the patient’s last name.
4. When a record is removed, a record locator will be inserted in its place.
5. Will file most recent documents on top of previous documents of the same type.
6. Will ensure patient name is on every document.
7. Will release medical record only with appropriate authorization.
8. Will maintain the confidentiality of patient information and medical records.
9. Will purge medical records if the patient has not been seen in three years.
10. Will comply with State and Federal Laws regarding subpoenas and court orders.
11. Will periodically perform chart review.
· Face to Face patient records request and Doctor to Doctor requests
· Sort incoming documents to send for providers signature
· Scan priority documents into the EMR
· Scan secondary documents into the EMR
· Receive and forward Patient Forms to Providers for completion
· Answer phones and field patient and incoming calls
· All duties assigned by management

Education/ Training:
- Degree: High School Diploma or GED or 1 or more years experience in general office duties.
Work Environment:
- The noise level in the work environment is usually moderate. The area is very busy.
Physical Demands:
The employee must frequently lift and/or move up to 25 pounds. While performing the duties of this job, the employee is regularly required to stand: use hands and talk or hear. The employee is frequently required to walk, bend, and reach with hands and arms.
Job Type: Full-time
Pay: $15.00 - $17.00 per hour
