Job Overview:
The Care Manager acts as the sole communicator between member and Plan. Responsibilities include creating and reviewing of person-centered service plan with member/members family, coordination of covered and non-covered benefits, and ensuring our members have a positive member experience. Care Manager is responsible for health education and ongoing communication with members, at minimum monthly.
Essential Responsibilities:
- Ongoing communication with members, dependent on needs, at minimum monthly.
- Follow up and respond to identified items requiring attention.
- Provide health education to members and work together to improve/stabilize health and minimize hospitalizations.
- Communication and coordination of member care with members and family members as well as vendor agencies.
- Provide excellent member experience and follow up on any member concerns or needs.
- Timely and thorough documentation in members’ charts.
- Participates in team meetings and supervision sessions.
- Other duties as identified as a necessary part of employee’s role.
Specific Knowledge, Skills, and Abilities:
- Knowledge of member assessment procedure.
- Knowledge of clinical review procedures.
- Knowledge and understanding of health education.
- Demonstrates ability to effectively communicate information with members of all ages and socioeconomic levels.
- Ability to read and interpret documents. Ability to speak effectively with members and employees of the agency.
Required Education, Experience, and Licenses:
- The appropriate background in health care and/or long term care.
- RN or Master’s Degree in Social Work.
- Bilingual preferred (Spanish, Chinese, Arabic, Hindi, Bengali).
- Hybrid schedule (3 days in office & 2 days remote).
Compensation:

MSW - $34.61 to $39.42
