University Home Care Inc is seeking a dedicated OT Home Care professional to provide high-quality occupational therapy services in the comfort of patients’ homes in the Southeast Michigan Region. Must be willing to travel. The ideal candidate will be passionate about helping patients improve their daily living skills and regain independence through personalized care plans.
- Conduct thorough assessments of patients’ functional abilities and needs in home settings.
- Develop and implement individualized occupational therapy treatment plans.
- Provide hands-on therapy, education, and support to patients and their families.
- Monitor patient progress and modify treatment plans as necessary.
- Document all patient interactions, progress notes, and treatment plans accurately and timely in compliance with company policies and regulatory standards.
- Collaborate with healthcare professionals and caregivers to ensure comprehensive patient care.
- Maintain professional licensure and keep abreast of latest occupational therapy practices.
- Current and unrestricted Occupational Therapist (OT) license in the state of Michigan.
- Proven experience in home care or outpatient occupational therapy settings preferred.
- Strong interpersonal and communication skills to effectively engage with patients and families.
- Ability to work independently and manage a flexible schedule.
- Valid driver’s license and reliable transportation to travel to patient homes within the Southeast Michigan Region.
- CPR certification is preferred or willingness to obtain.
- Competitive hourly compensation ranging from $70.00 to $90.00 per visit
