Job Overview
We are seeking a dedicated and highly skilled Nurse Practitioner specializing in Long-term Care and Rehabilitation to join our healthcare team. This role involves providing advanced nursing services, managing complex patient needs, and collaborating with multidisciplinary teams to deliver exceptional healthcare within rehabilitation and long-term care facilities.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews, to develop individualized care plans.
- Prescribe medications and treatments in accordance with state regulations and collaborative practice agreements, ensuring safe medication management.
- Manage acute and chronic conditions such as geriatrics, dementia care, hospice & palliative medicine, and behavioral health issues.
- Perform procedures including suturing, IV infusion, catheterization, spinal taps, and dermal fillers when appropriate within scope of practice.
- Utilize electronic health records (EHR/Epic systems) for accurate documentation of patient encounters, treatment plans, and medical records management.
- Coordinate discharge planning, case management, and utilization review to optimize patient outcomes across inpatient, outpatient, and post-acute care settings.
- Engage in telehealth services to extend access to care while maintaining compliance with HIPAA regulations.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care tailored to individual needs such as ICU experience or Level I Trauma Center experience.
Experience
- Proven experience as a Nurse Practitioner with a focus on long-term care or rehabilitation environments.
- Demonstrated proficiency in managing complex cases involving critical care, acute pain management, dialysis, or infectious disease care.
- Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks; experience with Athenahealth is advantageous.
- Background in medical-surgical nursing, geriatrics, or hospital medicine; experience working within nursing homes or assisted living facilities is preferred.
- Knowledge of CPT coding, ICD-10/ICD-9 coding practices for accurate documentation and billing processes.
- Experience performing procedures such as suturing, venipuncture, phlebotomy, sterile processing techniques, and basic life support certification is required.
- Prior involvement in clinical research or teaching hospital environments enhances candidacy; familiarity with trauma center protocols is beneficial.

Pay: $144,644.00 - $187,200.00 per year
