Position Summary:
To work as part of the rehabilitation team in the local Bureau of Services for Blind Persons (BSBP) office, as a New Horizons employee. To assist the vocational rehabilitation counselor in consumer orientation and intake, and to assist in the implementation of the rehabilitation plan of the service delivery system.
Essential Duties/Major Responsibilities:
- Work with consumers assigned by the vocational rehabilitation counselor(s) at the MDCD/RS office in providing assistance to youths and mental health consumers with disabilities.
- Assist the vocational rehabilitation counselor in performing intakes and conducting orientation sessions. Include gathering diagnostic information, assisting with the completion of forms of required paperwork.
- Assist in implementing the rehabilitation plan and provide follow up to be sure that the consumer has attended appointments and meetings as indicated in the plan. To include researching information for consumers and assistance with implementation of program objectives within the office setting and community.
- Participate in consumer activities and attends meetings as directed by the rehabilitation counselor.
- Will transport consumers to appointments if necessary.
- To provide assistance with vocational exploration and job development. Assisting consumers with applications, interviews, and general orientation to job seeking skills. Assists with job acquisition and provides follow-up, as well as develops positive relationships with employers.
- Maintain case records as appropriate to individual consumers. Maintain case notes and documentation as required, and use of computer system as requested.
- To abide by the policies and procedures of Michigan department of Career Development/Rehabilitation Services and New Horizons Rehabilitation Services, Inc.
- Uphold stated agency rules, regulations and policies, including confidentiality of consumer information.

Pay: $25.00 - $30.00 per hour
