Position Summary:
We are currently seeking a licensed and motivated Physical Therapist Assistant (PTA) to join our growing home health team. Under the supervision of a licensed Physical Therapist, the PTA will provide therapy services to patients in their homes to help improve mobility, strength, and independence.
Responsibilities:
- Provide in-home physical therapy treatments as directed by the supervising PT
- Assist in the development and implementation of individualized care plans
- Monitor patient progress and report changes to the PT and care team
- Educate patients and caregivers on exercises and mobility techniques
- Document services provided and maintain accurate records
- Communicate effectively with team members, patients, and families
- Travel to patient homes within your assigned territory
- Educate patients and their families about exercises, care plans, and discharge planning to ensure continuity of care.
- Collaborate with healthcare professionals to coordinate patient care and optimize treatment outcomes.
- Maintain accurate records of patient treatments and progress using medical terminology.
- Support geriatric patients by adapting therapy techniques to meet their specific needs.
Skills
- Current license as a Physical Therapist Assistant (PTA) in the state of California
- Minimum of 1 year of experience in a clinical or home health setting preferred (new grads welcome to apply!)
- CPR certification
- Valid driver’s license and reliable transportation
- Strong communication and organizational skills
- Ability to work independently and manage time effectively

Pay: $90.00 - $110.00 per hour
