Social Worker Responsibilities:
The responsibilities of the position include providing social work services to patients and families and to assure adequate and timely coverage of patients.

POSITION REQUIREMENTS FOR THE PER DIEM SOCIAL WORKER:
- Valid LMSW
- Must have a car
- Excellent interpersonal and communication skills.
- Understanding of the psycho-social issues related to families in crisis and end of life issues.
- Experience and competency to perform a psycho-social assessment and intervene appropriately.
- Experience and competency to work in the home or facility setting.
- Must be able to handle multiple tasks, be flexible and prioritize work load.
- Must be highly organized.
EXPERIENCE AND EDUCATION FOR THE SOCIAL WORKER:
- At least one year in a health care setting required, preferably home care or hospice
- Prior clinical experience working with geriatric, oncology or terminally ill populations highly desirable
