Overview
The Care Coordination Manager is responsible for coordinating client care services within the Partial Hospitalization Program (PHP), maintaining a limited caseload of up to six (6) clients, and cultivating strategic relationships with referral sources and community partners. This positions serves as a key liaison between the organization, clients, families, healthcare providers, and community stakeholders to promote continuity of care, enhance community engagement, and support program growth (case management duties).
Duties
Care Coordination and Client Services
- Maintain a caseload of up to six (6) PHP clients to ensure continuity of care and direct engagement with clinical services.
- Conduct assessments and identify client needs, barriers, strengths, and resource requirements.
- Collaborate with the interdisciplinary treatment team to support treatment planning and service coordination.
- Coordinate referrals and linkages to outpatient providers, medical services, community resources, housing supports, vocational services, and other relevant resources.
- Facilitate admission, transition, discharge, and aftercare planning activities.
- Communicate with clients, families, guardians, referral sources, and treatment providers as appropriate.
- Monitor client progress and address barriers to treatment participation and successful discharge.
- Participate in treatment team meetings and clinical case consultations.
Community Outreach and Relationship Development
- Develop, maintain, and strengthen relationships with referral sources, hospitals, behavioral health providers, primary care providers, community agencies, schools, employers, and other stakeholders.
- Serve as a primary ambassador for the PHP program within the community.
- Conduct outreach visits, presentations, networking activities, and educational meetings with referral partners.
- Promote awareness of organizational services and admission criteria.
- Establish and maintain a referral tracking process to monitor outreach effectiveness.
- Collaborate with leadership to identify opportunities for strategic growth and partnership development.
Program Development and Quality Improvement
- Identify service gaps and community needs that may inform program development.
- Assist in the implementation of initiatives designed to improve client access, engagement, and retention.
- Participate in quality improvement activities and performance monitoring.
- Provide feedback to leadership regarding referral trends, community needs, and partnership opportunities.
Documentation and Compliance
- Complete all required documentation accurately and within established timelines.
- Maintain records of outreach activities, referral contacts, care coordination services, and client interactions.
- Ensure compliance with HIPAA, accreditation standards, payer requirements, and organizational policies.
- Participate in audits and quality assurance activities as requested.
Experience
- Proven management experience in healthcare settings with a focus on Mental health/SUD therapy and case management
- Background in behavioral health counseling or mental health services with an understanding of behavioral therapy techniques is advantageous.
- Minimum of 2 years experience in Behavioral Health, care coordination, case management, community outreach or related services
- Experience in PHP, IOP, crisis services, inpatient behavioral health, or community mental health preferred
- Experience developing professional relationships with referral sources and community partners preferred
- Licensed clinican preferred- LCSW, LMFT, LMHC
