MDS Coordinator
Twin Oaks Rehab and Nursing, an 83-bed premier healthcare provider in Danvers, MA, is seeking an exceptional MDS Coordinator to join our team. This position offers competitive compensation of $90,000–$125,000 annually, depending on experience and qualifications. As an MDS Coordinator, you will play a critical role in ensuring accurate resident assessments, maximizing reimbursement opportunities, and supporting the delivery of high-quality resident care.

MDS Coordinator Key Responsibilities:
- Complete and oversee the MDS assessment process in accordance with federal and state regulations
- Ensure accurate and timely completion of MDS assessments, care area assessments (CAAs), and care plans
- Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members to ensure comprehensive resident assessments
- Monitor reimbursement opportunities and ensure proper documentation supports clinical services provided
- Assist with Medicare, Medicaid, and managed care documentation requirements
- Participate in quality improvement initiatives and regulatory survey preparedness
- Maintain knowledge of CMS guidelines, PDPM, and reimbursement regulations
MDS Coordinator Requirements:
- Experience with MDS assessments and care planning in a skilled nursing facility
- Strong understanding of PDPM, Medicare, Medicaid, and reimbursement processes
- Excellent organizational, communication, and analytical skills
- Ability to manage multiple deadlines while maintaining accuracy and compliance
Minimum Requirements (Must Have):
- Skilled Nursing Facility (SNF) experience
- Prior MDS experience
- Working knowledge of CMS regulations and reimbursement systems
- Experience coordinating interdisciplinary care plan meetings
Preferred Qualifications:
- RAC-CT certification (preferred but not required)
- Current licensure as a Registered Nurse (RN) in the state of Massachusetts
- Experience with PointClickCare or similar long-term care software
