Position Schedule:
This position is for a 2:00 PM – 10:00 PM shift, including weekends and holidays as needed.
What You Will Do:

Provide direct resident care services while following individualized plans of care for all residents.

Administer medications following all standards of practice, state regulations, and community policies, including required documentation, reporting, and follow-up.

Perform resident assessments as required by state regulations and community policy under the direction of the Director of Assisted Living.

Observe residents for changes in condition and report any concerns immediately to the supervisor and/or Director of Assisted Living.

Document all pertinent observations, communication with residents’ responsible parties, clinical procedures, and daily care by the end of each shift.

Complete and maintain individualized care plans for each resident.

Perform other duties as assigned by leadership.
Supervisory Responsibilities:
This position works under the supervision of the Director of Assisted Living and/or Director of Memory Care but is expected to perform independently and exercise good judgment. The role may assist with supervision, direction, and review of other associates’ work and may participate in performance evaluations and disciplinary actions.

The ideal candidate will have:

High school diploma or equivalent (GED) required.

Current Licensed Practical Nurse (LPN) license in the applicable state required.

Current CPR certification required.

At least one (1) year of related work experience preferred.

Ability to sit, stand, walk, climb stairs, bend, stoop, crouch, kneel, reach overhead, and perform fine motor tasks throughout the workday.

Ability to lift up to 25 pounds unassisted and assist with transferring, repositioning, pushing, and pulling residents using proper equipment and assistance as needed.
