Job Summary:
We are looking for a motivated LVN who loves patient care and is ready to make a real impact in home health. You’ll provide skilled nursing services, support patient independence, and work closely with a small, friendly team.
Key Responsibilities:
- Provide skilled nursing care per physician orders and individualized care plans.
- Administer medications, injections, and treatments safely.
- Monitor patient conditions and communicate changes promptly.
- Educate patients and families on disease management and self-care.
- Maintain accurate documentation in compliance with regulations.
- Collaborate with the team to ensure excellent patient care.
Qualifications:
- Current LVN license in California
- Experience in home health preferred
- Strong communication and interpersonal skills
- Reliable transportation to visit patients’ homes
- Passion for patient-centered care
Pay: $45.00 per hour
Work Location: In person
