We are currently seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our healthcare team in Greenville, TX. As an LVN on our team, you will provide high-quality nursing care to patients, assist with care coordination, and support remote monitoring initiatives to improve patient outcomes. - Provide direct nursing care to patients in accordance with established protocols and standards.
- Assist with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, including patient education and monitoring.
- Document patient information accurately and timely in electronic health records.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
- Monitor patient conditions and report any changes or concerns promptly.
- Support patient engagement and adherence to treatment plans through effective communication.
- Current and valid LVN license in the state of Texas.
- Experience with Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) preferred.
- Strong clinical assessment and patient care skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a team.
- Familiarity with electronic health records (EHR) systems.
