Build strong relationships with residents and their families.
Coordinate care plans and interdisciplinary services.
Conduct resident assessments and routine follow-up visits.
Collaborate with nursing, recreation, dietary, housekeeping, and leadership to ensure exceptional resident care.
Assist residents and families in navigating services and available resources.
Maintain accurate documentation and regulatory compliance.
Advocate for residents while promoting their safety, independence, and well-being.
Participate in care plan meetings and interdisciplinary team discussions.
Help create an environment where every resident feels valued and heard.
Compassionate, resident-focused professional
Excellent communication and problem-solving skills
Strong organizational and documentation abilities
Experience in Assisted Living, Long-Term Care, Social Services, Case Management, or Healthcare preferred
Ability to build trusting relationships with residents, families, and team members
