Per Visit Rate:
- Revisit = $36.00 per Visit
Job Summary: Community Visiting Nurse Association is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Home Health team. This is a per diem, fee-for-service position providing one-on-one patient care in the home setting.
Job Description:
- Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.
- Monitor patient status and report changes in condition to the RN and/or physician.
- Assist with implementing and updating individualized care plans.
- Review medications and reinforce medication education.
- Collaborate with the interdisciplinary team to ensure quality patient care.
- Educate patients and caregivers on disease management, medications, and safety.
- Document care and patient progress in the electronic medical record.
- Participate in case conferences and care planning discussions.
- Follow agency policies, infection prevention standards, and regulatory requirements.
Requirements:
- Current New Jersey Licensed Practical Nurse (LPN) license.
- Valid driver's license and reliable transportation.
- One year of nursing experience.
Preferred:
- Home health, hospice, long-term care, or hospital experience.
