Position Summary
La Paz Hospice is seeking a dedicated and compassionate Hospice Registered Nurse to join our team on a per diem or full-time basis. The ideal candidate will possess a strong background in various clinical settings, including critical care, emergency medicine, and post-acute care, with a commitment to providing exceptional patient-centered care.
As a Hospice Registered Nurse, you will be responsible for delivering comprehensive nursing services, coordinating patient care plans, and ensuring comfort and dignity for patients during their end-of-life journey. Your expertise in medical assessment, symptom management, and interdisciplinary collaboration will be vital to our mission of compassionate hospice care.
Responsibilities:
- Conduct thorough patient assessments to evaluate physical, emotional, and spiritual needs in accordance with hospice protocols.
- Develop and implement individualized care plans that address symptom management, pain control, and comfort measures.
- Manage medication administration and monitor patient responses to ensure safety and efficacy.
- Collaborate with physicians, social workers, chaplains, and other healthcare professionals to coordinate comprehensive care delivery.
- Provide education and support to patients and families regarding disease progression, treatment options, and end-of-life planning.
- Document all patient interactions accurately within electronic health records (EHR) systems while maintaining compliance with HIPAA regulations.
- Assist with discharge planning and facilitate transitions between inpatient, outpatient, or post-acute settings as needed.
- Ensure adherence to health regulation policies and quality standards while maintaining a compassionate approach to patient service.
Skills/Experience:
- Experience working within hospice settings provides valuable skills for managing acute symptoms and complex cases.
- Knowledge of infectious disease care, geriatrics, behavioral health, and medical-surgical nursing is preferred.
- Familiarity with electronic health records (EMR) management systems and medical terminology is essential for accurate documentation.
- Prior experience managing patients as a nurse case manager or in utilization management roles is advantageous.
- Demonstrated ability to perform comprehensive patient assessments including vital signs, physiology evaluation, and diagnostic evaluation techniques.
- Strong understanding of healthcare policies related to managed care, discharge planning, and hospital regulations ensures compliance and quality assurance.
Qualifications:
- Registered nurse with current licensure to practice professional nursing in the state of California.
- Graduate of National League for Nursing accredited school of nursing.
- Maintain a current CPR certification.
- Valid driver’s license & insured vehicle
- Minimum of two years’ experience, at least one of which is in the area of public health or home care nursing is preferred.
