We are seeking a compassionate and adaptable Registered Nurse (RN) to provide high-quality care in a Home Health and Hospice setting across theThomas, Brooks, and Colquitt county areas. This role is ideal for a nurse who thrives in dynamic environments, values patient-centered care, and is comfortable working independently in patients’ homes.
Job Description:
Job Title: RN Home Health
Location: Thomasville, GA
Schedule: 5x8 hrs
Shift: Days
Travel Pay Range: $2300 - $2526 weekly gross DOE
Local Pay Range: $1920 - $2200 weekly gross DOE
Key Responsibilities
- Deliver skilled nursing care to home health and hospice patients in accordance with individualized care plans
- Perform comprehensive patient assessments, including physical, emotional, and psychosocial needs
- Administer medications, treatments, and interventions as prescribed
- Monitor patient progress and adjust care plans in collaboration with the interdisciplinary team
- Provide education and emotional support to patients and their families/caregivers
- Maintain accurate and timely documentation in compliance with regulatory standards
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Participate in on-call rotation and respond to patient needs as required
- Travel between patient homes; mileage reimbursed between visits (first to last patient)
Requirements:
- Active Registered Nurse (RN) license
- Previous hospice experience (preferred but not required)
- Strong clinical assessment and critical thinking skills
- Valid driver’s license and reliable transportation
- Oasis experience required
- Homcare Homebase experience required
