Physical Therapist (PT) – PRN (Town Hall Estates Hillsboro)

Starting rate at $55.00 per hour, with increases based on experience and qualifications.
We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team on a PRN basis. We are looking for a clinician who is committed to helping patients regain independence, improve mobility, and achieve meaningful functional outcomes.
Our therapy team provides high-quality, person-centered care in a collaborative environment. We focus on individualized treatment plans to support residents in reaching their highest level of function and improving quality of life.
Responsibilities:
- Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders
- Administer skilled physical therapy treatments to improve mobility, restore function, reduce pain, and promote healing
- Monitor patient progress and modify treatment plans as clinically appropriate
- Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support optimal patient outcomes
- Educate residents, families, and caregivers on treatment plans, exercises, and safety techniques
- Maintain accurate and timely documentation in accordance with facility and regulatory requirements
- Participate in care planning meetings and interdisciplinary team discussions as needed
Qualifications:
- Active Texas Physical Therapist license
- Graduate of an accredited Physical Therapy program
- SNF or Long-Term Care experience preferred, but not required
- Experience with Net Health documentation system is a plus
- Strong communication, clinical reasoning, and patient care skills
