Job Overview
We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, delivering high-quality patient care across diverse community settings. In this vital role, you will provide skilled nursing services directly in patients’ homes, assisted living facilities, nursing homes, and other outpatient environments. Your expertise will support individuals with complex health needs, including those with chronic conditions, developmental disabilities, or recovering from acute illnesses. Responsibilities
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications, injections, IV infusions, and perform catheterization with precision and adherence to safety protocols.
- Manage complex cases involving dementia care, Alzheimer's care, memory care, hospice care, and acute pain management.
- Utilize electronic health record (EHR) systems such as Epic or Cerner to document patient interactions accurately and efficiently.
- Provide education and coaching on health maintenance, medication management, nutrition, and disease prevention tailored to individual needs.
- Coordinate discharge planning and collaborate with multidisciplinary teams for post-acute care or long-term management.
- Ensure infection control standards are maintained during all procedures including sterile processing and aseptic techniques.
Qualifications
- Valid LVN license in good standing with current state certification requirements.
- Proven experience in critical care settings such as ICU or Level I Trauma Centers preferred.
- Strong background in medical-surgical nursing, geriatrics, emergency medicine, or home & community care.
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly desirable.
- Knowledge of ICD-9/ICD-10 coding for accurate medical documentation and billing purposes.
- Experience working with individuals with disabilities or developmental challenges is a plus.
- Demonstrated ability to perform venipuncture, phlebotomy, tube feeding management, and airway management techniques.
