Position Summary
The Assistant Clinical Nurse Manager supports the day-to-day clinical operations of the home health agency, ensuring high-quality, patient-centered care is delivered safely and in compliance with regulatory standards. This role partners closely with field nursing staff, interdisciplinary teams, and agency leadership to drive clinical excellence and operational efficiency.
Key Responsibilities
- Supervise and mentor field nursing staff, including RNs, LPNs, and home health aides
- Conduct clinical chart reviews and OASIS assessments to ensure accuracy, compliance, and quality outcomes
- Assist in hiring, onboarding, and performance evaluations of clinical staff
- Manage patient caseloads, care coordination, and scheduling to ensure continuity of care
- Serve as a clinical resource for staff, resolving complex patient care issues in a timely manner
- Support infection control, quality assurance, and patient safety initiatives
- Ensure compliance with federal, state, and Joint Commission/accreditation standards
- Participate in on-call rotation as needed
Qualifications
- Active RN licensure in the state of practice (BSN preferred)
- Minimum 3 years clinical nursing experience; home health experience strongly preferred
- Prior supervisory or charge nurse experience required
- Proficiency with EMR systems and OASIS documentation
- Strong leadership, communication, and problem-solving skills
