Job Details / Responsibilities:
Provide clinical pharmacy services in an inpatient hospital setting
Review and verify medication orders for appropriateness
Collaborate with physicians, nurses, and interdisciplinary teams on patient care plans
Monitor patient therapy for efficacy and safety, including pharmacokinetic dosing
Participate in code responses and emergency situations as needed
Educate patients and staff on medication use and safety
Support hospital formulary management and medication safety initiatives
Work a mix of day and evening shifts with every other weekend rotation
Qualifications:
Doctor of Pharmacy or BS Pharmacy degree required
Minimum of 2 years of pharmacist experience
Hospital experience preferred
Pharmacy state licensure required within 60 days of start date
BLS or ACLS required
PALS required for pediatric practice
