Pay: $45,988.26 – $55,383.71 per year

What You’ll Do

Provide emotional, psychosocial, and practical support to hospice patients and families.

Assess patient and family needs and develop appropriate care plans.

Provide counseling, crisis intervention, and grief support as needed.

Connect patients and families with community resources and services.

Advocate for patient and family needs, dignity, and quality of life.

Collaborate with nurses, physicians, chaplains, aides, and other members of the hospice team.

Maintain accurate and confidential documentation in compliance with HIPAA and hospice standards.

Qualifications

A degree in Social Work or a related field is required; a Master’s degree is preferred.

Appropriate social work licensure or credentials as required.

Hospice, healthcare, or social work experience preferred.

Strong communication, organizational, and interpersonal skills.

Compassionate, dependable, and able to work independently as part of an interdisciplinary team.

Ability to travel to patient locations as needed.
