Post Rehab is seeking a Physical Therapist to join our rehabilitation team in a skilled nursing setting. This role offers the opportunity to work with residents focused on improving mobility, strength, and overall functional independence.
Responsibilities:

Perform physical therapy evaluations and establish individualized treatment plans

Provide therapy focused on gait training, transfers, balance, strengthening, and mobility goals

Collaborate with rehab and nursing staff regarding resident care plans

Monitor progress and update treatment approaches as needed

Maintain accurate and timely documentation

Qualifications:

Active PT license required
