Job Summary
We are seeking dedicated and compassionate Nurses to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and demonstrate proficiency in various nursing practices. As Nurses, you will be responsible for providing high-quality nursing care in various settings, including critical care, senior care, and home community care.
Positions Open
- PM: 2:00pm - 10:30pm 4 days on 2 days off
Responsibilities
- Conduct & follow individualized care plans.
- LPN conduct collect data on assessments for patient needs.
- Follow RN develop individualized care plans.
- LPN Administer medications and treatments as prescribed.
- Monitor patient progress and respond to changes in condition, ensuring adherence to established standards of care.
- Collaborate with interdisciplinary teams to facilitate discharge planning and coordinate ongoing patient care.
- LPN provide education to patients and families regarding health management and treatment options.
- Maintain compliance with HIPAA regulations and ensure the confidentiality of patient information.
- Participate in critical care transport as needed, ensuring safe transfer of patients.
- Uphold the highest standards of nursing practice while delivering compassionate care.
Skills
- Strong understanding of HIPAA regulations and commitment to patient confidentiality.
- LPN proficient in catheterization techniques and critical care protocols.
- Experience in home community care settings is preferred.
- Knowledge of senior care practices and the unique needs of elderly patients.
- Familiarity with discharge planning processes and systems for effective patient transitions.
- Excellent communication skills for effective collaboration with healthcare teams and patient education.
- Strong organizational skills with attention to detail in maintaining accurate patient records.

Pay: $27.00 - $30.00 per hour
