Job Description
We are seeking a highly organized and motivated Patient Care Coordinator to manage patient intake, referral coordination, and scheduling for our home health agency. This role is critical to ensuring timely admissions, efficient scheduling of clinicians, and excellent communication with patients, families, and referral sources. Working Hours Monday to Friday 8.30am to 5.00 pm
Required Qualification
- Bachelor’s degree in one of the following or related fields: Healthcare Administration & Health Information Management
- Strong organizational and multitasking abilities
- Excellent communication and customer service skills
- Ability to work in a fast-paced healthcare environment
- Basic computer proficiency (Microsoft Office, EMR systems)
- Current Administrative experience, preferably for home health or other healthcare settings.
- Knowledge of medical terminology or a general medical background desirable.
- Adaptable to working in a fast-paced environment.
Preferred Qualification:
- Experience in home health, hospice, or healthcare office setting
- Knowledge of medical terminology and insurance verification
- Experience with staff scheduling or care coordination
- Familiarity with EMR systems (e.g., WellSky, Homecare Homebase)
Essential Functions:
- Retrieves and routes incoming calls in a professional manner and provides customer service assistance as appropriate.
- Receive and process incoming patient referrals from hospitals, physicians, and community partners.
- Review referrals for eligibility, documentation, and service needs.
- Coordinate patient intake and admissions (start-of-care setup).
- Schedule nurses, therapists, and caregivers based on patient needs and staff availability.
- Verify insurance information and patient demographics
- Communicate with patients, families, and referral sources regarding services and scheduling
- Follow up on pending referrals to maximize admission conversion
- Collaborate with clinical and administrative teams to ensure smooth operations
- Order and track various office and medical supplies.
- Verify patient insurance benefits coverage to ensure medical procedures are covered.
- Run weekly reports to ensure timely delivery and follow-up on supply orders.
- Tracking physician orders to obtain physician signatures
- Perform other administrative duties as assigned.
