Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Bilingual Registered Nurse (RN) for Element Care PACE in Lynn, Massachusetts.
POSITION OVERVIEW
-
Provide comprehensive nursing care for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).

Perform initial and ongoing nursing assessments in both clinic and community settings.

Develop, implement, and evaluate individualized care plans that promote independence and reduce hospitalizations.

Collaborate with physicians, therapists, social workers, and the interdisciplinary care team to coordinate participant care.

Educate participants and caregivers regarding disease management, medications, and preventative health.

Maintain accurate clinical documentation within the electronic medical record.

Competitive salary from $80,000-$104,000 based on experience.

Requirements:
MINIMUM REQUIREMENTS
-
Active Registered Nurse (RN) license in Massachusetts.

Bilingual in English and Spanish.
-
Minimum two years of recent Registered Nurse experience.

Ability to provide nursing care in both clinic and home/community settings.

Strong assessment, communication, and documentation skills.
-
Experience in primary care, geriatrics, home health, or community health is preferred.
-
Valid driver's license and reliable transportation.
