Case Manager – Elect One Case Management LLC; Under Community Long Term Care (CLTC)
Job Type: Part-Time to Start
Pay: $35-40 per productivity based on experience
Schedule: Monday–Friday ~ Must be available during the hours of 8:30 am -5:00 pm
Overview
The Case Manager will coordinate for assigned Office of Intellectual and Developmental Disabilities (OIDD) participants in accordance with SCDHHS.
This role ensures participants receive appropriate, effective, and client-centered care by conducting face-to-face visits and phone communication monthly ie; (Initial, Visit Monthly Telephonic Contact, Quarterly Visits and Annual Re-Evaluation Visit).
The Case Manager will develop strong relationships with Participants, Providers and SCDHHS division leaders, while maintaining compliance with federal, state, and third-party payor regulations.
Key Responsibilities:
Provide case management services including assessment, planning, implementation,
monitoring, and evaluation.
Coordinate care that is safe, timely, effective, equitable, and client-centered.
Manage case assignments, document individualized service plans, track case progress, and possible case terminations.
Advocate for clients by supporting informed decision-making regarding proffessional
care options.
Build and maintain effective working relationships with providers and care teams.
Document case information accurately, complete all required forms, and prepare reports.
Monitor and promote cost-effective interventions that improve outcomes.
Address psychosocial and motivational challenges impacting care.
Adhere to professional standards, agency policies, and regulatory requirements.
Required Skills & Competencies
Proven experience in case management (nursing, medical, mental health, care
management, or related field).
Strong knowledge of case management principles, healthcare management, and
reimbursement processes.
Experience addressing psychological and social aspects of care.
Excellent communication, organizational, and time management skills.
Strong problem-solving skills with the ability to multi-task.
Compassionate, professional, and a collaborative team player.
Required Qualifications
Bachelor’s degree in a health or human services field (i.e sociology, psychology, health
management, etc or vocational well-being) OR Licensed RN or LPN.
Minimum of two (2) years’ documented experience providing Care Coordination or Case
Management services.
Certification as a Case Manager (CCM or equivalent) is a plus.
Must be willing to complete required CLTC orientation.
*Must have reliable transportation to make visits to clients’ home, and home internet/wi-fi to complete reports electronically in a timely manner.
Tools to be successful will be provided along with monthly training.
