Position Overview:
Exeeda Wellness is seeking a compassionate and skilled Physical Therapist (PT) to provide outpatient physical therapy services for our growing primary care and wellness clinic in Annandale, VA. The ideal candidate will have experience working with older adults and patients with mobility limitations, chronic pain, balance issues, post-surgical rehabilitation needs, and age-related functional decline.
This role is especially suited for a patient-centered PT who is comfortable working with senior patients and communicating clearly with patients and families in both Korean and English. The PT will evaluate patients, create individualized treatment plans, provide therapeutic exercises and manual therapy as appropriate, document progress, and coordinate care with our primary care providers and clinical team.
Key Responsibilities
- Evaluate patients’ physical function, mobility, strength, balance, pain, and rehabilitation needs
- Develop individualized physical therapy plans of care based on patient condition, goals, and provider referrals
- Provide therapeutic exercise, gait training, balance training, functional mobility training, and patient education
- Provide manual therapy and other appropriate physical therapy interventions within scope of practice
- Work primarily with senior and geriatric patients in an outpatient clinic setting
- Support fall prevention, strengthening, mobility improvement, pain management, and functional independence
- Educate patients and families in Korean and English regarding exercises, safety, home programs, and treatment goals
- Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
- Coordinate with primary care providers and clinical staff regarding patient progress and care plans
- Maintain compliance with HIPAA, clinic policies, payer requirements, and professional documentation standards
- Maintain a clean, safe, and professional treatment environment
- Escalate clinical concerns, changes in condition, or safety risks to the appropriate provider
Required Qualifications
- Active Physical Therapist license in Virginia
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Korean language proficiency required or strongly preferred
- Experience working with senior or geriatric patients
- Strong clinical reasoning, communication, and documentation skills
- Ability to work independently and collaboratively in an outpatient clinic setting
- Compassionate, professional, and patient-centered approach
Preferred Qualifications
- 2+ years of outpatient physical therapy experience
- Experience with Medicare and older adult populations
- Experience with balance training, fall prevention, gait training, chronic pain, orthopedic conditions, and post-surgical rehabilitation
- Experience working in primary care, outpatient rehabilitation, senior care, or wellness clinic settings
- Familiarity with EMR/EHR documentation (Practice Fusion)
- Interest in helping build and grow a physical therapy program within an established clinic
