Madison County Service Coordination (MCSC) is seeking an experienced and motivated professional to lead our team of Support Coordinators. Essential Duties and Responsibilities
- Supervise and provide leadership to a team of Support Coordinators.
- Provide ongoing consultation, guidance, training, and performance feedback to staff.
- Participate in the selection, onboarding, training, evaluation, and performance management of supervised employees.
- Monitor the development and implementation of Individual Support Plans and ensure compliance with Missouri Department of Mental Health requirements.
- Review and approve Individual Support Plans and related documentation.
- Conduct quality assurance reviews of case records, documentation, and service coordination activities.
- Assist in planning, assessing, and determining program needs for individuals receiving services.
- Maintain positive working relationships with service providers, community agencies, families, and stakeholders.
- Help coordinate staff meetings, training sessions, and mentoring opportunities for Support Coordinators.
- May maintain a limited caseload and assist with intake of new individuals into services.
- Prepare reports and maintain accurate records.
- Exercise independent judgment in coordinating services and resolving client and staff concerns.
- Stay informed of community resources, service options, and best practices related to developmental disabilities.
- Perform other related duties as assigned.
Requirements
- Bachelor's degree from an accredited college or university in a human services field, or a Registered Nursing License.
- Minimum of five (5) years of experience as a TCM Support Coordinator.
- Strong knowledge of case management methods, developmental disabilities, community resources, and support services.
- Ability to establish and maintain effective working relationships with staff, families, providers, and community partners.
- Valid driver's license and dependable transportation.
- Ability to pass required background screenings and drug testing.
