WHAT WE'RE LOOKING FOR
- Active New York RN License.
- home health or home-based nursing experience.
- Experience conducting comprehensive patient assessments.
- Experience with UAS-NY, OASIS, CHHA, LHCSA, MLTC, hospice, or community-based care is highly preferred.
- Strong clinical assessment and documentation skills.
- Excellent verbal and written communication.
- Ability to work independently and manage a field-based caseload.
- Comfortable using technology and electronic documentation systems.
- Strong organizational skills and attention to detail.
