Qualifications

Bachelor's degree in social work, Human Services, Education, Public Health, or a related field OR equivalent professional and community experience

2+years of experience working with elementary and/or middle school youth in educational, community-based, or youth development settings.

Strong relationship-building and engagement skills with youth, families, community partners, and staff.

Demonstrated leadership, organizational, and program coordination skills.

Strong verbal and written communication skills.

Ability to provide and receive constructive feedback.

Commitment to youth development, prevention services, and organizational values.

Availability to work Saturdays and occasional evenings as required.

Proficiency with documentation, data collection, and reporting requirements.

Ability to maintain confidentiality and exercise sound professional judgment.

TB Test Clearance within the last two years, as required for programs located on school district sites.
Under the supervision of the Director of Youth & Families Programs, the Family Program Specialist supports the development, coordination, and delivery of youth and family prevention programs funded through the San Francisco Department of Public Health. The position focuses on fostering positive youth development, family engagement, leadership development, and successful transitions between school-year and summer programming. The Teen Program Specialist collaborates with students, families, schools, and community partners to facilitate evidence-based services, provide case management support, and ensure program goals, compliance requirements, and participant outcomes are achieved.
Program Coordination & Implementation

Coordinate, facilitate, and implement evidence-based youth and family prevention programs in alignment with grant, curriculum, funding, and Department of Public Health requirements.

Design and implement transition, enrichment, and leadership development opportunities for youth. Support the delivery of Strengthening Families Program (SFP), LifeSkills Training (LST), and other approved prevention curricula.

Plan and manage program schedules, logistics, activities, and deliverables.

Facilitate workshops, groups, and activities that promote positive youth development and family well-being.

Attend required staff meetings, trainings, and professional development activities.
Youth & Family Engagement

Recruit, engage, and retain program participants and families.

Build and maintain positive relationships with students, families, community members, and partner organizations.

Promote participant engagement and support achievement of program outcomes.

Maintain participation levels in accordance with contract and program requirements.
Case Management & Direct Services

Conduct assessments and develop Family Service Plans using appropriate intervention strategies.

Participate in Case Management Team meetings and coordinate services as assigned.

Support students and families through referrals, resources, and ongoing engagement.
Collaboration & Community Partnerships

Collaborate with school personnel, the school Beacon program, community-based organizations, and Department of Public Health partners.

Participate in required meetings, trainings, and community events.

Foster partnerships that enhance services and support for youth and families.
Documentation, Compliance & Reporting

Maintain accurate participant records, attendance, case notes, and program data.

Complete and submit all required reports and documentation in a timely manner.

Ensure compliance with agency, school district, contractual, and Department of Public Health requirements.

Track and report program outcomes and participation data.
Work Environment & Physical Demands
This position is primarily based in a school and community-based setting and requires regular interaction with youth, families, school personnel, and community partners. The employee must be able to perform the essential functions of the position with or without reasonable accommodation, including the physical, visual, and auditory abilities necessary to facilitate programs, conduct meetings, complete documentation, and engage with participants.
This position may require standing, walking, sitting, speaking, hearing, and using standard office equipment for extended periods. Occasional lifting, carrying, or moving of program materials and supplies weighing up to 25 lbs. may be required.

Salary Description
The Family Program Specialist position offers a compensation range of $27.00 - $33.00 per hour. This rate is based on salary benchmarking for similar roles and is aligned with grant fund approvals and requirements for the position.
