Overview
Provide compassionate nursing care to terminally ill patients in their homes, nursing facilities, or hospice centers. Under the supervision of a Registered Nurse (RN) or physician, the LPN assists with the implementation of the plan of care, monitors patients’ conditions, administers medications, and offers support to patients and their families during end-of-life care.We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients, ensuring their comfort and well-being while adhering to established protocols and standards. This role requires a strong commitment to patient care, effective communication skills, and the ability to work collaboratively within a multidisciplinary team.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Perform patient assessments and monitor vital signs.
- Assist in the development and implementation of individualized care plans.
- Provide care for patients with diverse needs, including those with developmental disabilities and infectious diseases.
- Implement aseptic techniques in all procedures to prevent infection.
- Support patients in behavioral health settings, ensuring a safe and supportive environment.
- Educate patients and their families about health management, disease prevention, and self-care techniques.
- Maintain accurate patient records and documentation in compliance with regulatory standards.
- Collaborate with healthcare professionals to coordinate patient care effectively.
Sklls
- Hospice care ; Knowledge of aseptic techniques and infection control practices.
- Familiarity with hospice care a plus.
- Strong understanding of behavioral health principles and practices.
- Ability to provide comprehensive care for patients with infectious diseases.
- Experience with Durable Medical Equipment (DME) management is desirable.
- Excellent communication skills, both verbal and written, for effective patient interaction and documentation.
- Strong organizational skills to manage multiple tasks efficiently while maintaining attention to detail.
