Job Overview
We are seeking a proactive and detail-oriented Case Manager to join our team. In this vital role, you will support a wide range of Home Health Provider Agency functions. Your energetic approach and meticulous attention to detail will help foster a positive work environment, streamline Case Manager processes, and enhance employee engagement.
Duties
- Manage data entry , ensuring accuracy and confidentiality
- Design, execute, and adjust the Plan of Care (POC) alongside physicians, therapists, and the patient's family.
- Maintain accurate clinical notes and adhere to Medicare or private insurance reimbursement criteria.
- Supervise and delegate duties to Home Health Personal Attendants.
- Assist with talent acquisition activities, scheduling interviews, and supporting onboarding processes
- Support employee orientation and facilitate new hire process
- Coordinate employee evaluation and performance management processes in collaboration with managers.
- Maintain clients records in compliance with state requirements
- Proven experience in customer service
- Excellent communication skills to effectively interact with employees at all levels
- Ability to handle sensitive information with discretion and maintain confidentiality
- Demonstrated organizational skills with the ability to manage multiple priorities efficiently
- Bilingual
Pay: From $13.00 per hour
