Currently seeking a provider for a kind active 60 year old gentleman who wants to reside in the Greater Portland area, preferably on the Metro bus line. He has a wide array of interests, including watching classic television programs, visiting friends, traveling, swimming, walking/hiking, dining out, seasonal endeavors, bowling, and spending quiet peaceful time in his own space.
Potential providers should currently reside in the Greater Portland area, have their own home or apartment with one bedroom dedicated to the client, and/or be willing to relocate to the area on very short notice. A situation with a separate space would be ideal. A garage apartment, attic space, basement apartment, and/or attached or semi-attached living space would increase the likelihood of long term success. This gentleman does enjoy regular peaceful undisturbed quiet time.
Also seeking other Providers for additional folks seeking the shared living model of residential support.
What are the qualifications?
- Providers may be an individual, couple, family, or the parent of the individual
- Providers may rent or own their home and be 21+ years of age
- Must completed state required trainings and pass a background check (all adults 18+ residing in the home must pass a background check)
- Must be able to work with team members and service agencies
- Must have the flexibility to support the individual they are supporting
- Must provide a supportive home and inclusion in the community
- Maintain a clean and healthy living environment
- Provide daily support and care for the individual served per the person-centered plan which will be provided.
- Provide daily documentation to support the ongoing need for services.
