Hospital Liaison — Northern Suburbs
ALIYA Healthcare is seeking an experienced Hospital Liaison to serve as the vital link between ALIYA Healthcare facilities and partnering hospitals in the northern suburbs. This individual will ensure smooth communication, exceptional care coordination, and timely transitions for our residents. Responsibilities:
Conduct daily visits to hospitals where ALIYA Healthcare residents are admitted across multiple northern suburb facilities
Monitor the condition and progress of hospitalized residents and communicate updates to facility staff
Serve as the primary point of contact for families of hospitalized residents, providing timely updates and addressing concerns
Collaborate with hospital discharge planners and case managers to facilitate smooth, well-coordinated discharge plans
Coordinate with ALIYA facility staff to prepare for residents' return, ensuring a seamless transition back to the facility
Advocate for residents' needs and preferences throughout their hospital stay
Maintain accurate records of all hospital visits, interactions, and follow-ups
Attend internal meetings with ALIYA Healthcare staff to report on hospitalized residents' statuses and address any issues or concerns

Qualifications:
Minimum of 1 year of experience as a Hospital Liaison required
Strong understanding of post-acute care, skilled nursing, and hospital discharge processes
Excellent communication and interpersonal skills with the ability to build relationships with hospital staff, families, and facility leadership
Highly organized with the ability to manage multiple facilities and cases simultaneously
Compassionate, professional demeanor with a strong commitment to resident advocacy
