Duties
- Dispense prescriptions accurately and efficiently, ensuring compliance with all pharmacy laws and regulations.
- Conduct comprehensive patient assessments to determine appropriate medication therapy, including pediatrics and acute care cases.
- Collaborate with medical teams to develop and adjust medication plans, including pain management and immunizations.
- Prepare and compound medications using aseptic techniques, including IV infusions and other sterile preparations.
- Manage inventory levels meticulously to prevent shortages or overstocking, utilizing pharmacy software and EMR/EHR systems.
- Provide patient education on medication administration, potential side effects, and proper usage to enhance adherence and outcomes.
- Perform patient care activities such as medication reconciliation, medication administration oversight, and patient monitoring within hospital settings.
- Maintain strict adherence to HIPAA regulations and ensure all documentation aligns with medical terminology standards.
- Support pharmacy technicians by overseeing their workflow, ensuring accuracy in medication compounding and inventory management.
- Participate in medical management initiatives, including review of clinical protocols related to critical care, pediatrics, and acute care environments.
Skills
- Proven expertise in LTC (Long-Term Care) pharmacy operations and hospital pharmacy settings.
- Critical care experience with a strong understanding of physiology, anatomy, and medical terminology relevant to acute patient management.
- Completion of a pharmacy residency program with specialized clinical pharmacy experience preferred.
- Demonstrated proficiency with pharmacy software systems, EMR (Electronic Medical Records), and EHR (Electronic Health Records) platforms.
- Skilled in compounding medications aseptically, including IV insertion and infusion techniques for complex therapies.
- Knowledge of immunizations administration protocols and vaccination procedures.
- Strong background in medication safety practices such as aseptic technique, inventory control, and medication administration procedures.
- Experience with pain management protocols within hospital or acute care environments.
- Excellent patient service skills with the ability to assess individual needs effectively through patient assessment techniques.
- Familiarity with HIPAA compliance standards to safeguard patient privacy at all times.
