MDS Coordinators work to improve patient care and help long-term care facilities maintain federal funding. To do that, they complete several crucial tasks throughout the day. Some duties and responsibilities for an MDS Coordinator will include:
- Completing and updating MDS forms on all patients in the care facility
- Performing clinical assessments to gather information on residents
- Developing care plans for patients to coordinate available services
- Submitting updates and paperwork to maintain compliance
- Participating in the admission and discharge processes
- Conducting and attending meetings with other care team members
- Investigating complaints or issues from residents and their families
- Educating patients and their families
The MDS Coordinator at Western Sky Community Care Center will also support the Director of Nursing Services. Expected qualifications include:
- Understanding of Medicare and Medicaid requirements
- Familiarity with the MDS assessment tool, including MDS coding knowledge
- An understanding of medical terminology, nursing practices and patient care standards
- Attention to detail to ensure precision in completing assessments and accurate data entry
- Assessment, data collection and documentation skills
- Written and verbal communication skills
- Organizational skills
- Comfort with electronic records
Pay: $40.00 - $43.00 per hour
