Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs.
- Manage and coordinate patient care plans, including discharge planning, post-acute care, and outpatient services, ensuring seamless transitions across care settings.
- Perform diagnostic evaluations and assist with medical procedures in various environments such as home health visits, clinics, or hospital settings.
- Collaborate closely with healthcare professionals—including physicians, therapists, and case managers—to develop and implement effective treatment strategies.
- Utilize electronic health records (EHR/EHR) systems to document patient information accurately while maintaining strict compliance with HIPAA regulations.
- Provide patient education on health management, medication adherence, infection control, and disease prevention tailored to individual needs.
- Engage in case management activities such as clinical documentation review, utilization management, and managing patients as a nurse case manager to optimize resource use and improve outcomes.
Experience
- Proven clinical experience in critical care environments such as ICU or emergency medicine with demonstrated proficiency in managing acute conditions.
- Experience working within Level I or Level II trauma centers is highly desirable, showcasing your ability to handle complex trauma cases efficiently.
- Background in hospital medicine or outpatient clinics with exposure to medical-surgical units, geriatrics, pediatrics (PICU experience preferred), behavioral health, or hospice care.
- Strong knowledge of health regulation policies including HIPAA compliance and medical terminology essential for accurate documentation and patient confidentiality.
- Skilled in using EMR/EHR systems for documentation, intake assessments, discharge planning, and medical records management.
- Adept at performing patient assessments involving vital signs monitoring, diagnostic evaluation, and clinical case management across inpatient and outpatient settings.
- Excellent client communication skills with the ability to educate patients effectively while collaborating with multidisciplinary teams to ensure comprehensive care delivery.
