We are currently looking to add a Per Diem RN Field nurse to our team. This person will join a dynamic group, who strive to provide exceptional service to our clients in Lower Westchester County, in the comforts of their own homes. Responsibilities and Duties
- The initial client assessment to include patient assessment and the home environmental assessment
- Develops the plan of care and performs supervisory visits in the client’s place of residence.
- Daily case management, including compliance with all NY LHCSA regulatory standards
- Supervision of paraprofessional staff
- Delivery of quality aide services along with support in the recruitment and retention of staff
Qualifications and Skills
- RN currently registered and licensed to practice nursing in NY
- Experience with NY state LHCSA regulations
- Experience in dealing with Dementia is a plus
- Previous home care and hospice care
- Experience in Home Health Aide training
- The applicant should be able to demonstrate maturity, professionalism and the ability to deal effectively with the demands of the job
- Must have excellent telephone and interpersonal skills.
- Must be able to navigate difficult / challenging situations with professionalism
- Must have some office administrative experience, specifically filing and organizational, with both paper and electronic files.
- Excellent computer and MS office skills.
- The applicant will be required to submit a complete background check, including a check of criminal and driving records along with finger printing and substance testing where required
- The applicant must have Valid Drivers License and a safe and reliable car
- The applicant will be required to provide physical/PDD results within past 3 months.
Job Type: Part-time
Pay: $50.00 per hour
